The Problem with the RICE Procotol

The most recommended protocol for healing in western medicine is the RICE protocol – rice, ice, compression, elevation. Most of us have used the RICE method, or at least some component of it, to help improve our recovery. It’s been ingrained into our brains, especially if you play sports or stay active. The interesting thing is, after talking to the experts and reading the literature it turns out the protocol is wrong. The Harvard trained doctor who developed RICE has since recanted his statement saying that ice not only delays healing, but also causes additional damage.

Why is Ice so Harmful?

Muscle damage is an inevitable part of all sports and physical activities. Congestion builds up around the damaged site after exercise. The quicker you can get rid of the waste, the quicker you can recover. Using ice after a workout traps congestion and prevents the natural flow of oxygen and supplies to the area. Even worse, sitting with a bag of ice on your arm won’t just delay the healing process, it will make the situation worse. Ice causes additional swelling that wouldn’t have normally occurred.

Why Did Ice Become a Staple for Baseball Players?

Icing became popularized for baseball players thanks to a photo that was taken of Dodger’s pitching legend, Sandy Koufax, with his arm in a bucket of ice. All the world saw was the star player icing his arm, thinking that must be how he keeps his arm healthy. In reality, the story is much different, as told from those who were there when it happened.

Koufax’s arm was in pain and he refused to take any more needles. He told the training staff to figure out a different way to stop the pain. To comply, athletic trainers put a wetsuit-like sleeve on his arm and stuck it in ice. Thus, the myth began that putting ice on your arm after throwing was somehow going to make you recover. However, Sandy was using ice for his arm pain. No doubt, ice can make the pain go away temporarily, but it doesn’t facilitate healing.

So if ice is wrong, then what should you be doing to improve recovery? The short answer is low-stress movement, better known as active recovery.

How it Works

When you workout, there’s a lot going on within your body; chemical reactions are occurring, your muscles are breaking down, and you’re becoming more and more fatigued. These conditions signal to your body to begin the recovery process.

Below are the pad placements that Trevor Bauer typically uses to recover in between innings and after the game.

In the Dugout:

For convenience and time purposes, the elbow is the easiest spot to target when using Marc Pro in the dugout. Trevor places a pad on his inner elbow, outer elbow, forearm, and bicep. Once the inning is over, he takes the pads off and goes back out to pitch. Keep in mind this protocol does require a little bit of extra time (15-20 seconds), which means fewer throws on the mound, but for him it’s worth it since he feels a lot better throughout the game and doesn’t get nearly as sore.

After the Game:

Shoulder recovery is a big one for baseball players. During the game, it’s hard to reach the shoulder when there’s a jersey and sweat in the way. Icing has become one of the most controversial topics in the sports medicine world. For decades, icing damaged tissue – from sore muscles to significant injuries – was the go-to treatment method. However, research has found that icing not only delays the healing process, it also potentially causes additional damage. Eric Cressey, President of Cressey Sports Performance and the Director of Player Health & Performance for a major league team, interviews the “Anti-Ice Man” to get to the bottom of this recently popular debate on the repercussions of ice/RICE for tissue damage and what the science says is the most effective way to recover.

Podcast Notes – To Ice or Not to Ice?

Before getting any further, it should be clear that “tissue damage” or “tissue trauma” can result from a spectrum of causes, which could be anywhere from muscles breaking down during training to more significant injuries. But, regardless of where it falls on the spectrum, the recovery process is the same.

With tissue damage, it’s important to act immediately. The quicker you can manage your recovery, the quicker you can get back to training and competing. Plus, mismanaging your recovery can also cause unnecessary further damage.

Icing has been a popular treatment for tissue damage for years. It became even bigger when Dr. Gabe Mirkin coined the RICE protocol and made it a standard of care. However, what would you think if you knew he later publicly recanted his statements saying that research shows he was clearly wrong? In 2015 he did just that, saying that icing delays healing and causes additional damage.  

Tissue Preservation & Regeneration

There are two important components to recovering damaged tissue, preservation and regeneration. Unfortunately, ice does not help with either. When damage occurs, the first step is to preserve the surrounding healthy tissue that was not affected by the initial trauma. The only way to do that is to decongest the area. If the damaged area is not decongested, healing is compromised and healthy cells that were not involved in the initial trauma will likely die.

How do you decongest the area? The only path to get the waste out of the body is through the passive lymphatic system. Since the system is passive, it relies on muscle activation in and around the damaged site to push the waste out.

The second step is tissue regeneration. The good news is that the muscle activation that’s required to decongest the area, also helps in the regeneration phase. Muscle activation helps regenerate tissue by creating sprouting angiogenesis and restoring circulation to the damaged area. Plus, tissue loading also prevents disuse atrophy, and reorganizes the repaired tissue – which ultimately helps prevent adhesions. As you can see, it’s muscle activation that drives recovery and allows the body to optimally repair.   

Popular “Recovery” Techniques

There are four things that need to happen for recovery to take place: bring good stuff in, get back stuff out, produce and release myokines, and repair and reorganize tissue. If your technique isn’t addressing those, it will not be effective for the purpose of recovery.

Icing: this technique does not help with any of these stages of recovery and using ice delays healing and may even cause additional damage. For more details on why not to use ice for recovery, check out this whitepaper.

“If you sit still with a bag of ice wrapped tightly around the area, you literally trap the waste in and around the damage site and prevent the natural flow of oxygen supplies.”

Foam Roller: the purpose of these are to break adhesions and cause tissue damage, so while they may be effective for that, they will not facilitate muscle recovery.

Soft Tissue Mobilization (ie. Hawk Grips): the intent for these tools is also to cause tissue damage, not recover.

Compression Boots: since squeezing the skin does not involve any tissue loading, there is no significant impact on recovery. A small amount of waste may be able to be pushed out through external pressure, however, only superficial vessels will be affected. Without any tissue stress, there is no increase in circulation, the lymphatic system is not activated, and myokines are not produced.

So then what is the most effective way to recover? The answer is active recovery.

How to Effectively Recover

When you have damaged tissue, it may seem counterintuitive to move the area. But, the key here is to create low-stress muscle activation that doesn’t cause pain. The best way to do that is through the use of Marc Pro, which creates non-fatiguing movement. If you want your body to have a positive reaction to training and become more effective than when you started, arm care is a must. Pitchers of all levels are adopting the notion that training doesn’t stop on the field. Throwing takes a toll on the tissues, so in order to prevent overuse injuries and keep arms healthy, recovery and warm-ups are becoming the norm. Kelly Starrett shares a few solutions for pre and post arm care that will help throwers survive and thrive in the season.

1. Press Downs

Healthy elbows require nutrition and hydration, which are provided by your blood. One of the easiest ways to increase elbow tissue perfusion rates, is by incorporating press downs. Engorging your elbows with blood is a useful technique to use before you start to throw to properly prepare tissues or as soon as you’re done throwing to help recover the area. The goal with these press downs is to increase blood into the elbow, without causing fatigue.

2. VooDoo Floss

When it comes to getting athletes turned around and ready for the vigor of sport, Kelly Starrett has had excellent success across all levels of baseball with using VooDoo floss. This strategy involves high-level compression, making the vasculature work hard to push blood into the area. But, once the band is removed, you’ll get a big rebound effect and the blood will come crashing back in.

Note: VooDoo floss traps the entire fascial sheets of the elbow and the forearm into a single mass and restricts blood flow, so after a couple minutes wrapped up, your elbow may not feel very good. Work as much as you can with your window of opportunity. By rotating the tissue system and the bone system underneath it, you can reach areas that are normally not addressed.

3. Marc Pro

The last technique that Kelly is a huge fan of is Marc Pro, which speeds the recovery process and allows you to recover while you’re sitting around or on the car ride home. One of the problems with traditional heavy arm loading, like throwing, is that it’s difficult to decongest the area afterwards with enough non-exercise activity. Marc Pro helps reset the clock to zero, so instead of waiting 2-3 days for your body to adapt to the stress, you can do it right away.

To place the pads, Kelly recommends placing one pad below and one above the area you want to recover and decongest. Just make sure the target area is contracting. Injury Prevention Tips for Baseball Pitchers

1. Warm Up

Warming up before throwing is an important step in injury prevention. During a warm up, blood flow increases in your body, increasing the temperature of your muscles. When muscles are warmed, their elasticity increases which reduces muscle tension and allows the muscles to contract efficiently. Including a warm up prior to pitching can also help improve your range of motion. Decreased rotation and flexion can lead to an increased risk for elbow injuries, especially at the pro level.

2. Strengthen

Going from 0 to 100 once the season starts up again is almost a sure fire way to get an injury. At the pro and college level, many players get injured during the progression from off season to in season training. While taking a break is essential, it’s recommended to maintain strength and gradually increase workload while transitioning into training. This will help prevent overuse injuries. When muscles, tendons, and ligaments are strong, they can take on more activity and won’t be as likely to break down.

3. Recover

Because overuse is so strongly related to baseball pitching injuries, recovery has to be an essential part of every pitcher’s training routine. From youth athletes to pro pitchers, every level of players are at risk for overuse injuries. Recovery techniques should be used after every game or throwing session. Most importantly, athletes should make sure they are fully recovered before pitching again. Over 200 professional pitchers and all 30 MLB teams use Marc Pro to recover and prevent injuries.
